If you would like to order your merchandise RIGHT NOW, just use the forms next to each item in the catalog to select color or print, quantity, and size, and add them to your shopping cart. You can continue shopping, adding items to your cart, then check out, using our secure payment page to pay by credit card.
2.PRINT FORM.

If you prefer, you may print out the form below and fax it to us at 1-877-203-5550. We will contact you by phone to arrange for your package to ship the very NEXT DAY, and to accept payment for your order.

RETURN AND REFUND POLICIES: Your Uniform Warehouse is happy to accept any merchandise for return or refund within 15 days of purchase, for any reason, with a return authorization number clearly marked on box. You must call our office to receive the authorization number BEFORE return-shipping the items. We are sorry but shipping charges, as well as return-shipping charges, are not refundable.

We cannot accept returns that have been worn or washed. NO EXCEPTIONS.
